Program Mission:

To facilitate direct, easy access to information on health care services, programs, and resources for individuals, seniors, families, and care givers.

Community Resource Center

Our resources are free of charge. Call or email us to make an appointment.

• independent and assisted living
• insurance coverage and government programs such as Medicare Part D and HICAP
• transportation, medical equipment
• support groups, hospice

Our volunteer staff is ready and able to help you navigate through the process of finding health services.

Resource Guide

Brochures, listings and contact information of care providers, hospitals, special programs, nutrition and health tips are available for you in a take-home folder.  

Free Workshops

Mid-week talks are offered at our foundation office to explain what services and resources are available within the community and how to access them.Please visit our events page on our web-site for the schedule or call our office.
